The family of a client who is dying asks the nurse what place would be best for the client if the family want to request the services of hospice. What should the nurse explain to the family about hospice?
 
  1. Hospice can see the client at their institution.
  2. Hospice only treats clients in the home.
  3. Hospice will treat the client wherever the client is located.
  4. Hospice treats clients in long-term care facilities.

The client with cancer is dying, and the physician has written a do-not-resuscitate (DNR) order. The client develops respiratory distress because of choking on oral secretions. The nurse should:
 
  1. Sit with the client until death.
  2. Notify the charge nurse immediately.

1. DNR orders refer to resuscitation efforts by using CPR, medications, and mechanical support. The nurse would not allow the client to die, because choking from secretions is not what the DNR order is referring to. The client is kept comfortable and afforded a dignified death.
2. The charge nurse would be notified if the nurse could not clear the airway.
3. Dying from choking on secretions would not be part of a DNR order. The nurse ensures the client's comfort and focuses on meeting needs, one of which is an open airway.
4. The nurse who leaves the room at this time is abandoning the client.
