The client's nodding may simply indicate hearing the information being provided. The nurse should ask questions to assess understanding. Asking the client if she understands is a closed question and may not determine adequate information. The client's signature on the discharge paperwork may be indicated as a part of the discharge plan but is not the next step that should be taken by the nurse.
