An UPDATE or DELETE without a WHERE clause can effect every record in a table. An UPDATE can change all the values to be the same as the SET value. The DELETE could remove all records from a table. The way to prevent it is to make sure that UPDATE and DELETE queries have clear and precise conditions in the WHERE Clause.
