Include in your outline behavioral changes you would recommend, foods to avoid with appropriate substitutes, and foods to include. Explain why you are making changes.

Answer to Question 1

Mr. G had leg cramps while he was taking Lasix. His kidneys were functioning at that time. Lasix causes K to be excreted via the kidneys and blood K values drop. A symptom of decreasing K is leg cramps. Bananas and/or orange juice taken daily can help prevent this. If not, K supplements can be prescribed.

Answer to Question 2

I would make arrangements to meet with Mr. G and his wife. It is important for her to understand the necessity for Mr. G's diet. I would teach the new diet to both. Before you can expect a married man to follow a diet, it is extremely important that he and his wife understand why he should be on the diet. I would teach them the foods to eat and to avoid as outlined in questions 16 and 17 . Printed materials should be provided with the instruction. Videotapes, slide presentations, etc., are also helpful. Retention will be greater if the patient reads, hears, and watches the diet instruction.

There are numerous behavioral modifications that could be introduced. Some would be: not eating before going to bed, eating in only the dining room, breakfast room, or kitchen (no eating in the den, bedroom, etc.), having someone else serve you, and many more. At the end of the instruction the patient should be presented with a work phone number to call for further information after getting home. The more interest taken in the patient, the greater the chance of compliance.
