Imple-mentation of the TCM has been associated with the following favorable outcomes: 1) reductions in preventable hospital readmissions; 2) short-term improvements in physical health, functional status, and quality of life; 3) increased overall satisfaction with the care experience; and 4) re-ductions in total health care costs.
