Answer to Question 1

C
Disease prevention aims to prevent specific diseases by avoiding, limiting, or restricting certain behaviors. This negative focus may discourage people and make them feel deprived.
Considering higher levels of wellness would be health promotion, not disease prevention.
People are often motivated to make changes but lack information and concrete plans to do so. Certainly, the nurse would not want to assume a negative view of patients as being generally unmotivated.
The nurse should assess whether or not the information is too difficult for patients, but for the majority of patients, this will not be the case.

Answer to Question 2

B, C, E
PICOT stands for population, intervention, comparison, outcome, and time.
