As death approaches, a client terminally ill with AIDS tells the nurse I really do not want to see a
  lot of visitors anymore. Just my parents and Jim can come in for a while each day.. What action
  should the nurse take?
 
  a. Discuss this with the parents and Jim and suggest that they explain the client's
  decision to his friends.
  b. Ask the client to reconsider his decision because he has many interested friends.
  c. Suggest that the client discuss his wishes with his physician.

As many clients approach death, they begin to withdraw. In the stage of acceptance, many clients are
exhausted and tired, and interactions of a social nature are a burden. Many prefer to have someone
present at the bedside who will sit without talking constantly. Options B and C imply the client
should revise his position. Option D could keep out the visitors the client actually wants to see.

Answer to Question 2

A
Hypnotics can worsen existing sleep disturbances when they induce drug-dependency insomnia.
Once the drug is discontinued the individual may have rebound insomnia and nightmares.
Neuroleptics rarely worsen insomnia. CNS stimulants worsen insomnia while they are in use.
Tricyclic antidepressants may help insomnia.
