Answer to Question 1

A
The client with moderate Alzheimer's disease has memory loss that begins to interfere with
activities. This client may be able to use environmental cues such as labels on doors to compensate
for memory loss. Option B: Regular toileting may be helpful, but a 2-hour schedule is often more
reasonable. Option C: This intervention is more appropriate as a later stage intervention. It is
probably unnecessary at this point. Option D will be more relevant when the client demonstrates
hyperorality.

Answer to Question 2

D
The milieu that is predictable and permits clients to behave autonomously as much as possible is less
likely to be the scene of violence than units with authoritarian staff, little structure, haphazard
routines, or permissive limits about directing anger at staff. Less structured routines create situations
in which clients feel slighted, ignored, and insecure, all feelings known to underlie anger. Direct
venting of anger at staff often leads to escalation.
