Question 2

The theory developed for WC was that the proton inhibitor decreased the possibly already reduced absorption of B12, causing PN. When blood was analyzed, B12 was in the normal range but, considering the large doses that WC had been taking for several weeks without taking Nexium, it was assumed that B12 should have been higher. A clue that this was correct was the elevation of folic acid. If B12 is low, why would folic acid be higher than normal? (Hint: look up the methyl folate trap).

Answer to Question 1

Uses: mild to moderate depression, anxiety, and as an antiviral. Side effects/interactions: photosensitivity, fatigue, delayed hypersensitivity, serotonin syndrome, N&V. If taken with tyramine containing foods, it could cause a hypertensive crisis. If taken prior to surgery, it could increase the effects of anesthesia. Can also interfere with some medications, such as cyclosporine.

Answer to Question 2

The theory of the methyl folate trap simply states that as B12 decreases, folate increases to compensate.
