Calcium acetate is the most effective phosphate binder for ESRD patients. Aluminum hydroxide gels used to be the treatment of choice years ago. Explain why these gels are no longer recommended for ESRD.

Question 2

Another medical problem that patients with ESRD have to contend with is renalosteodystrophy . Relate why this is a concern.

Answer to Question 1

Aluminum hydroxide gels contain aluminum and this is not extracted from the blood very well by any of the dialysis regimens. Aluminum toxicity results and produces symptoms similar to Alzheimer's disease.

Answer to Question 2

As previously mentioned, renal failure prevents the activation 1,25-dihydroxycholecacciferol which results in calcium absorption problems and problems with calcium deposits in the bone. This is further complicated by elevated phosphorus in the blood. The end result is the promotion of renalosteodystrophy .
