What is considered to be good control for BS for someone with diabetes and what is considered poor control?

At 5 months of age, LV was receiving Similac Neocare at a rate of 8.4 kcals per hour. How many calories and how much protein was this providing? How many calories and how much protein should he be receiving at 5 months?

Answer to Question 1

According to Diabetes Care 28:S4-S36, 2005, glycemic control includes:
 A1C < 7.0
 Preprandial capillary plasma glucose 90130 mg/dl (5.07.2 mmol/l)
 Peak postprandial capillary plasma glucose < 180 mg/dl (< 10.0 mmol/l)
 Blood pressure < 130/80 mmHg
 LDL < 100 mg/dl (< 2.6 mmol/l)
 Triglycerides < 150 mg/dl (< 1.7 mmol/l)
 HDL > 40 mg/dl (> 1.1 mmol/l)
 Poor control is out of control, particularly on a regular basis. The higher the blood sugar, the poorer the control.

Answer to Question 2

8.4 x 24 = 201.6 kcals/day but this was the starting rate and not his final rate.
Similac Neocare = .74 kcal/cc
201.6/.74 = 272.4 cc/day
1L = 26 g of protein
.272 L x 26 = 7.1 g of protein

According to the RDAs, a normal infant at 5 months of age should be receiving approximately 650 kcals and 13g of protein. LV is receiving less than that but he is a premature infant that is much smaller than an infant that is 5 months born at term. Another method of recommending kcals is 120 - 150 kcals/kg/day for the healthy, growing premature infant. LV is receiving only 57.6 kcal/kg/day as an unhealthy infant. Also, this was his starting rate and not the final rate. Also, because of LV's BPD, fluids were a problem and he could not be given much more than this.
