Answer to Question 1

Answer: Proper identification of the patient; correct collection tube; proper storage, preservation and transport of the specimen; correct timing in making a blood smear.

Answer to Question 2

Answer: A second identifier, usually a hospital number or date of birth, should be on the specimen.
