The family of a client with a pacemaker asks the nurse what information would help them better care for the client at home. Family members appear to be anxious and fearful. What should the nurse plan?
 
  1. Have Home Health check the client daily.
  2. Delay the client's discharge.
  3. Suggest that the client live in a skilled nursing facility.
  4. Teach the family CPR.

A client has just been extubated after open-heart surgery. What should the nurse prepare the patient to begin?
 
  1. Oral fluids
  2. A soft diet
  3. Ambulation three times a day
  4. An incentive spirometer

1. Home health is not necessary for this client. The nurse can teach the family and client what information they need to monitor care at home.
2. The client's discharge does not need to be delayed.
3. If the client is functional, the nurse can teach the family and allay fears so that the client can live at home.
4. To help decrease anxiety in the client and family, the nurse should teach how to take a pulse, signs and symptoms of pacemaker failure, emergency procedures, and CPR.

1. The client who has just been extubated will continue to be n.p.o. until it is determined that the client is tolerating extubation.
2. The client will remain n.p.o. for a few days after extubation.
3. The client will not be walking until medications are stopped and the physician is sure the client is stable.
4. An incentive spirometer will help prevent lung collapse and improve oxygenation.
