Correct the following transcription errors:
 
  a. The patient was found to have nonobstructed azoospermia.
  b. Endometrial thickness is 13 mm trilaminar fibroid measuring 53 X 53 mm.
  c. Severe atheromatous plaquing of the distal aorta or bilateral iliac arteries.
  d. I will order non-invasive vascular studies and carotid Doppler.
  e. The patient's mother has no heart disease and has undergone angioplasty and stenting twice.
  f. Gallbladder ultrasound from All Saints Medical Center revealed cholecystolithiasis and no ductile dilatation.

Note: There are several ways this sentence can be corrected, but the key point is that trilaminar does not describe the fibroidit describes the endometrial (stripe) thickness.

Answer to Question 2
