Answer to Question 1

a. nonobstructive
b. Endometrial thickness is 13 mm, trilaminar, with a fibroid measuring 53 x 53 mm.
Note: There are several ways this sentence can be corrected, but the key point is that trilaminar does not describe the fibroidit describes the endometrial (stripe) thickness.
c. aorta and bilateral
d. noninvasive
e. known heart disease
f. ductal

Answer to Question 2

Based on the patient's current symptoms and objective findings, medications may be altered (increased, decreased, or discontinued). Changes to medication regimens are dictated in the Plan section of a report, so medication names that were dictated in the medication list may be repeated in the Plan.

The mixture of cocaine, saliva, and resin numbed the site enough to allow hours of drilling.
