The physician may have input into the care plan but does not have the right to make changes unilaterally, unless by virtue of changed orders (response B). The care plan is reviewed and revised in interdisciplinary team meetings and at other times deemed necessary by changes in patient status (response C). Response D is incorrect because the care plan is a valuable communication, planning, and evaluation tool.

Mr. Yost seems to be exhibiting classic withdrawal symptoms of depression that can be expected with his sudden loss of independence and self-care ability.
