The answers to questions concerning medications taken may signal possible drug-food interactions or reasons for loss of appetite. Finally, changes in weight may indicate overeating or undereating. The LPN may use a nutritional assessment tool if the place where she works has one, but if not, she should still ask the questions and determine the nutritional status of the person.
