A nutritional assessment should include finding out if the older adult has the financial resources to buy food, the ability to shop and cook, and details about what is eaten. The answers to questions concerning medications taken may signal possible drug-food interactions or reasons for loss of appetite. Finally, changes in weight may indicate overeating or undereating. The LPN may use a nutritional assessment tool if the place where she works has one, but if not, she should still ask the questions and determine the nutritional status of the person.
