Airway muscle weakness predisposes to aspiration and obstruction, and loss of expiratory muscle strength leads to ineffective cough and poor clearance of secretions. Weak inspiratory muscles are unable to generate adequate Vts, resulting in atelectasis and pulmonary shunt.
