The nurse should assess the client for major gaps in memory, such as difficulty recollecting the client's phone number during the mid-stage level of AD. Concentration, orientation, judgment, and planning abilities are severely affected during the mid-stage of AD. Disregarding or failing to recognize people, forgetting familiar words, and forgetting the car keys are seen during the early stage of AD.
