Answer to Question 1

1. Ask if the client would like to talk about stressors and problems.

Rationale:
Those who attempt suicide are overcome and overwhelmed with stressors in their lives. The nurse should ask the client to talk about her stressors and problems. The nurse should not ask the client about what caused the attempt at suicide. The nurse should also not try to distract the client by asking about school and activities. Suggesting the client read and listen to music may not be sufficient to reduce the client's stress.

Answer to Question 2

3. Widowed for 6 months

Rationale:
Of the assessment findings, the one that would increase the client's risk for suicide ideation or behavior is the recent loss of a spouse.. For the risk of age, elderly male clients have the highest risk followed by adolescents and college students. Active employment and engagement in activities does not increase the risk for suicide ideation or behavior.

An identified risk factor for osteoporosis is the intake of excessive amounts of vitamin A. Though Candida and Aspergillus species are the most common fungal pathogens causing invasive fungal disease in the immunocompromised, infections due to previously uncommon hyaline and dematiaceous filamentous fungi are occurring more often today. Rare fungal infections, once accurately diagnosed, may require surgical debridement, immunotherapy, and newer antifungals used singly or in combination with older antifungals, on a case-by-case basis.
