Answer to Question 1

ANS: 1
Myopia is nearsightedness. Peripheral vision is not reduced with myopia. The client with myopia is able to see close objects, but not distant objects. Peripheral vision may be decreased in open-angle glaucoma. Diminished night vision may occur with cataracts, not myopia. Problems with glare, flashes, and floaters may indicate eye disease and the client should be referred to a physician.

Answer to Question 2

ANS: 1
Clubbing of the nails is caused by a chronic lack of oxygen, such as occurs in heart or pulmonary disease. Paronychia is caused by local infection or trauma. Beau's lines are caused by systemic illness such as severe infection or by injury to the nail. Splinter hemorrhages are caused by minor trauma, subacute bacterial endocarditis, or trichinosis (infection by the roundworm).
