Mirtazapine blocks presynaptic alpha2-adrenergic receptors (which increase the release of norepinephrine), serotonin receptors, and histamine H1 receptors. Consequently, the drug decreases anxiety, agitation, insomnia, and migraine headaches as well as depression.

Thiamine deficiency is common in patients with alcoholism because of inadequate dietary intake and use of large amounts of thiamine to metabolize ethanol. Sodium, vitamin A, and vitamin K are important but not as crucial to this particular patient as thiamine.
