Answer to Question 1

Completed claim forms must be protected from view by anyone who is unauthorized to see patient information. Completed forms must be placed in a secure area, never in a nonsecure area, even when in envelopes. When errors are made on CMS-1500 claim forms and new ones must be printed, shred the forms with errors to protect patient privacy.
