Correct Answer: 1, 2, 5
Adverse effects of cyclophosphamide (Cytoxan) include hemorrhagic cystitis, renal failure, alopecia, stomatitis, and liver dysfunction. The nurse should encourage a daily fluid intake of 2 to 3 liters during treatment and offer suggestions to manage hair loss. However, spicy foods are not recommended for patients with stomatitis.

TIS means that the tumor is in situ. M0 means there is no evidence of distant metastasis. N1a means there is no metastasis to regional lymph nodes.
