The nurse is working with a client who is diagnosed with posttraumatic ttress disorder (PTSD). The client is preparing for discharge in a week. On what should the nurse focus with this client?

  2. Notify the physician when having nightmares.

  4. Eliminate all anxiety.

2. Nightmares will continue, and the nurse assists the client to cope with them.
3. The client with post-trauma syndrome is not aggressive.
4. Some anxiety is a protective mechanism in humans.

1. The client with anxiety has a difficult time focusing due to fear. The nurse plans communications with the client to be short and direct.
2. An anxious client would not benefit from an in-service, due to an inability to concentrate.
3. The client's fear is usually unreasonable and indefensible, but real to the client. Asking the client to justify the fear will increase the client's anxiety.
4. Fear is a symptom of a disorder. Having the client confront the fear will only increase the anxiety the client is feeling.
