1. Mild swelling is expected with burns.
2. Cool feet and absent pulses indicate that the circumferential burn injury has caused compression of the circulation, which is an emergency. The client will undergo an escharotomy to relieve the compression and restore circulation.
3. Leaking would occur from an open wound.
4. Stridor would indicate swelling of the airway.

1. Wound care is very painful and can cause nausea. Care should be given prior to meals.
2. There is no reason for the client to void first. Most clients with extensive burns are catheterized.
3. Giving pain medication will reduce the discomfort and the client's anxiety.
4. The client is already hydrated with IV fluids.
