Answer to Question 1

Answer: 3

1. Choosing appropriate food is not the goal of training the client to eat with a tracheostomy.
2. Ability to take medications orally is not the goal of nutrition. Before starting the client on oral feedings, the ability to swallow has been assessed.
3. The goal of nutrition is to maintain the client's weight.
4. Ambulation is not relevant to the client's ability to eat.

Answer to Question 2

Answer: 2

1. The nurse will monitor intake of fluids, but it is not the priority of care.
2. Because the tonsils are very vascular, the client is at risk for hemorrhage following surgery. The nurse should monitor the client closely for excessive bleeding.
3. The nurse would not be concerned about increased urine output unless the client was diabetic.
4. The client with an infection is not eligible for surgery until the infection is healed.
