Collaborative practice requires the discussion of diagnoses and management in the delivery of care. Case management involves one or more individuals overseeing the needs and requirements of a particular individual's health. Health promotion activities include disease prevention and healthy lifestyle interventions. Health education would be included in this particular situation, but collaboration is a more inclusive definition of what is occurring with these individuals and the care they require.
