Answer to Question 1

Answer: D

The nurse would question the order for a selective serotonin reuptake inhibitor (SSRI). This medication is used in the treatment of major depressive disorder (MDD) and dysthymic disorder, not seasonal affective disorder (SAD). All the other orders are appropriate for a client with SAD.

Answer to Question 2

Answer: C, D, E

Priority outcomes for the child who is dying are pain control and airway patency. The nurse would want to encourage the child in age-appropriate play depending on the condition of the child. It is not reasonable to expect the dying child to eat all meals, nor is it appropriate that the child not experience anticipatory grieving.
