Answer to Question 1

Answer: D

Clients with chronic conditions such as CHF should be further evaluated by the healthcare provider as to the cause of the fever. The nurse should not restrict the client's fluids. Providing warm blankets might make the client more uncomfortable. The client should not be encouraged to perform activity until the source of the fever has been identified.

Answer to Question 2

Answer: A

The very young and the very old have diminishing immunity, which places them at risk for fevers. Fevers are most frequently seen in children because of developing immunity. The nurse has no way of knowing if the client's immunity is compromised. The child may or may not be around people with illnesses. Common diseases of childhood frequently result in fevers.
