Risk for Ineffective Health Maintenance is the diagnosis with the highest priority for the client at this time. There is no evidence to suggest the client is not coping or is experiencing decisional conflict regarding the removal of the cataracts. The client does have disturbed sensory perception; however, the postoperative care is the highest priority at this time.

Some cataracts, present at birth or acquired, are due to retinopathy of prematurity, metabolic diseases such as galactosemia, or long-term use of corticosteroids. The nurse should explain that cataracts are seen in babies who are premature. Cataracts are not seen with most newborns. At one time, a major cause of congenital cataracts was congenital rubella syndrome; however, now that children and women receive rubella vaccine, there are very few cases of cataracts due to congenital rubella syndrome. Cataracts are not caused by an allergic reaction.
