Answer to Question 1

Correct Answer: 4

The UAP may help with routine care, such as bathing and feeding the client. The UAP should be taught what information needs to be reported while observing the client's status. Hanging dialysate fluid requires sterile technique, and should not be delegated to the UAP. Assisting with catheter insertion and emptying of the outflow fluid require assessment skills and use of sterile technique, which would not be appropriate for delegation to the UAP.

Answer to Question 2

Correct Answer: 3

Straw-colored outflow fluid is normal, and would not need to be reported. The remaining data indicate a problem, and the UAP should be instructed to report them to the nurse as soon as possible.
