Answer to Question 1

Correct Answer: 1,5,4,2,3

The nurse first must determine the medication to be administered, and then determine the proper route to administer it. Once the medication is prepared, the nurse must identify the client, using two methods of identification. When the nurse has verified the correct client, that person has the right to refuse the medication being offered. Whether or not the medication is accepted by the client, the nurse must document the interaction properly.

Answer to Question 2

Correct Answer: 1

The nurse must always use care to lock the drug cart before walking away because the cart contains syringes, needles, and numerous medications that could endanger a life. The client's MAR should have been retrieved prior to removing the medication from the cart in order to perform the rights of medication administration. Hand hygiene should be performed before touching a medication to be administered to a client. Documentation of medication administration should never be performed until the medication is given to the client.
