The nurse first must determine the medication to be administered, and then determine the proper route to administer it. Once the medication is prepared, the nurse must identify the client, using two methods of identification. When the nurse has verified the correct client, that person has the right to refuse the medication being offered. Whether or not the medication is accepted by the client, the nurse must document the interaction properly.

Documentation of medication administration should never be performed until the medication is given to the client.
