When two or more modifiers are necessary to completely define a service, the medical office assistant should:
 
  list each modifier on the same line as the CPT code, separated by commas.
  repeat the CPT code and append each modifier as many times as necessary.
  attach a special report instead of using the modifiers.
  append modifier -99 to the basic procedure code and list the other modifiers as part of the description of service.
