Answer to Question 1

Sex hormone binding globulin binds to estradiol and testosterone in the blood, allowing only a small amount to circulate as free molecules (unbound). Unbound hormones are biologically active; hormones bound to SHBG are not active. The bioavailability (activity) of estradiol and testosterone are influenced by the levels of SHBG. PCOS is primarily a syndrome of increased testosterone activity in females. Decreased levels of SHBG cause free testosterone levels to be higher and thus more bioactive, resulting in virilization in females.

Answer to Question 2

Progesterone balances the effects of estrogen. Unopposed estrogen is an excess of estrogen that is not balanced or kept in check by progesterone.
