Answer to Question 1

Like the diagnostic tests, the physical exam is used to make a diagnosis, so the exam results should support, or at least not contradict, whatever conclusions are dictated.

Answer to Question 2

The Review of Systems is a list of symptoms reported by the patient. Those symptoms should correspond to, or at least not contradict, the patient's current complaints.
