  1. Knowledge deficit related to hygiene
   2. Self-esteem issues related to personal appearance
   3. Insufficient nutrition related to skin breakouts
   4. All of the above would be appropriate nursing diagnoses for the teenager.

Question 2

The nurse recommends to the mother of a 9-month-old who has been exposed to Hepatitis A in daycare that she should:
 
  a. Get the hepatitis A vaccine now rather than waiting until the child is 1 year old
  b. Use airborne precautions in the home while caring for the baby
  c. Get passive immunity for the baby with immunoglobulin
  d. Treat the rash with calamine lotion

The child should be protected with immunoglobulin. Enteric precautions should be used. It is inappropriate to advance the time of inoculations. There is no rash associated with Hepatitis A.
