Answer to Question 1

4
Rationale 1: Insurance coverage is not a disadvantage with which the nurse would be concerned.
Rationale 2: Whether the patient is married is not a disadvantage for which the nurse would be assessing.
Rationale 3: Plans for working after the baby arrives is not an disadvantage for which the nurse would be assessing at this point.
Rationale 4: It is important for the nurse to question and assess for any chronic illnesses, such as diabetes, which are more common as one ages.

Answer to Question 2

3
Rationale 1: Increased blood pressure would cause vasoconstriction, and is not identified in this patient.
Rationale 2: Hypoglycemia would not be a usual assessment for the mother, but possibly for the infants.
Rationale 3: The nurse will assess for postpartum hemorrhage. This patient is at risk for hemorrhage due to overdistention of the uterus with twins and possible slower uterine involution.
Rationale 4: Postpartum infection would be assessed through lochia odor and uterine pain, and would not be expected this soon after delivery.
