Getting regular exercise is changing an exercise risk-factor behavior that could contribute to a health alteration. Immunization is disease prevention, not an altered risk factor. Medications to lower cholesterol would be a medical treatment for an existing health alteration. Changing dietary and exercise behaviors are examples of initiating change in risk factors to lower cholesterol.
