Answer to Question 1

A
Response:
Telling the parents to limit eating to meal and snack times is the best advice. This reduces the amount of exposure the child's teeth have to food. Urging them to take the child to see a dentist is sound advice but doesn't suggest actions they can take now to prevent caries. Carbohydrates react with oral bacteria to cause caries, but dietary intake should not be reduced. Avoiding fluoride toothpaste may help prevent fluorosis.

Answer to Question 2

D
Response:
It is essential to respect client autonomy and present information in a factual, nondirective manner. In these situations, the nurse needs to understand that the choice is the couple's to make. Gathering information for three generations obtains a broad overview of what has been seen in both sides of the family. Maintaining confidentiality of the information is as important as with any other client information gathered. Informing the family of the need for information in advance is necessary because of its personal nature.
