Groupthink is the consequence of extreme cohesion. Group members tend to isolate themselves from outside information, and critical thinking deteriorates because group members are reluctant to challenge the group's decisions. It can be avoided by: 1, promoting open inquiry, 2, using subgroups, 3, admitting shortcomings, and 4, holding second-chance meetings.
