These four steps are assessment and planning, communication, surveillance and supervision, and evaluation and feedback. When a nurse assesses the assistant's understanding of how the task is to be performed, when and what information is to be reported, and addresses any unique client characteristics and expectations, he or she is functioning under Step Two: Communication.
