High-risk delegation can lead to problems for the nurse. One concern to avoid this is to evaluate:
 
  a. if the delegated task could cause harm to the patient
  b. only tasks in the implementation phase of the nursing process
  c. if the NAP has completed the task before
  d. how well the licensed vocational nurse (LVN) functions in their role

The RN may be at risk if the delegated task can be performed only by the RN according to law, organizational policies and procedures, or professional standards of nursing practice; if the delegated task could involve substantial risk or harm to a patient; if the RN knowingly delegates a task to a person who has not had the appropriate training or orientation; or if the RN fails to adequately supervise the delegated activity and does not evaluate the delegated action by reassessing the patient (ANA, 1996).

Answer to Question 2

In considering the effectiveness of delegation, the nurse addresses whether the delegation was successful: was the task performed correctly, was the desired outcome achieved, what went well, and were there any problems or concerns. The availability of resources is not included in this evaluation process as the nurse is directly evaluating the delegation process to the NAP and the care the patient received.
