Social, epidemiological, and educational and organizational diagnoses are components of the precede phase. Implementation and outcome evaluation are components of the proceed phase.

During the action stage of change, the client engages in concrete actions to effect needed change. In the determination stage, the client decides there is a problem and is willing to make a change. In the precontemplation stage, the client does not think there is a problem and is not considering the possibility of change. In the contemplation stage, the client thinks there may be a problem, is thinking about change, and goes back and forth between concern and unconcern.
