Answer to Question 1

Correct Answer: 2

An open-ended question such as Where would you like to begin? gives the client permission to set the tone and direction of the conversation. The answer about the family health history would direct the conversation toward one specific topic. The answer about feelings is focused on the nurse's curiosity, not the client. Asking about why the client feels a certain way is too confrontational.

Answer to Question 2

Correct Answer: 2

DRGs are part of a classification system used to reimburse hospitals. Using DRGs, a hospital is paid a predetermined amount for a patient with a specific diagnosis. Nursing care plans are individualized based on Maslow's hierarchy of needs. Private health insurance premiums are monies charged to patients to provide insurance coverage selected by the patient. Physician consultations are made based on patient needs and not the established DRG.
