This question is open-ended and allows the client to share information about behavior that she may not even recognize as abuse. The other questions listed are more appropriate to ask as follow-up questions, once the woman has stated that she is being abused.

A caregiver smacking away a toddler's hand would not result in serious physical or emotional harm. All of the other answers would.
