It is an ongoing process that takes into account changes in the patient's condition and responses to interventions, which lead to a need for reassessmentor a return to the first step, as in a circular path. Response A is incorrect in that it does not address the need for continual reassessment and evaluation. Reponses B and D are incorrect because they are not individualized and do not relate to the needs of the individual patient.
