The nursing process includes the steps of assessment, nursing diagnosis, planning, intervention, and evaluation. It is an ongoing process that takes into account changes in the patient's condition and responses to interventions, which lead to a need for reassessmentor a return to the first step, as in a circular path. Response A is incorrect in that it does not address the need for continual reassessment and evaluation. Reponses B and D are incorrect because they are not individualized and do not relate to the needs of the individual patient.
